2. HIGH SCHOOL ATHLETIC PRE-PARTICIPATION EXAM FORM AND ATHLETIC CONSENT FORM & INSTRUCTIONS: These two forms must be filled out and returned to the Student Athletic Center (SAC) at Uni High. You CANNOT particiate in any of the summer camps if your forms are not on file at the SAC. You can contact the SAC at 936-7784. The forms are little different. 9th & 11th graders need to have a physical. Please select the proper grade level for your forms.

3. USA WATER POLO »» All players must have an active USA Water Polo membership to participate in the summer camp. Click on the above link to visit the USA Water Polo website. Find the “Join Now” link. You can sign up for the "Silver" membership. If you are not already registered with a club, please join our High Schools club called “University Water Polo Club". Our Club Number is #1.
